Assessment Terms and Conditions

These terms and conditions apply to workplace needs assessments and all diagnostic assessments offered by Flourishing Minds Limited, including assessments for dyslexia, dyscalculia and related specific learning difficulties, for children aged 8 and above, young people and adults.

Nature and Outcome of the Assessment

The purpose of a diagnostic assessment is to develop a detailed and accurate understanding of the individual’s learning profile. An assessment does not guarantee that a diagnosis of dyslexia, dyscalculia or any other specific learning difficulty will be identified.

Regardless of the outcome, all assessments result in a comprehensive written report outlining strengths, areas of difficulty, conclusions and personalised recommendations. The assessment fee reflects the professional time involved in preparation, assessment, scoring, analysis and report writing, rather than the confirmation of a specific diagnosis.

Workplace Needs Assessments are needs‑based assessments designed to identify practical workplace adjustments and support strategies. They are not diagnostic assessments and do not constitute a medical, psychological or occupational therapy diagnosis. Recommendations are advisory and based on the information available at the time of assessment.

Booking and Payment

Assessment appointments are booked via the online booking system, where clients select an available date and time. A £100 non‑refundable deposit is required at the time of booking to secure the appointment and is deducted from the total assessment fee.

The remaining balance (including any additional agreed travel fees) must be paid in full at least 24 hours before the assessment takes place. Payment details will be provided following booking confirmation.

Where services are commissioned by an employer or third party, responsibility for payment must be agreed in advance.

Prior to the Assessment

Prior to the assessment, you will be asked to complete a background information questionnaire. Where the individual being assessed is a child or young person, an additional questionnaire may be requested from a teacher, SENCo or other professional who knows them well.

For adults, background information will be gathered directly from the individual and, where appropriate, from relevant documentation such as previous educational records or workplace information.

For Workplace Needs Assessments, background information will be gathered from the individual and, where appropriate, from job descriptions or workplace information, with the individual’s consent.

 

All required questionnaires and paperwork must be returned prior to the assessment date. Without sufficient background information, a diagnostic decision may not be possible and the report cannot be completed.

It is essential that the individual being assessed has had an eyesight check within the 12 months prior to the assessment. If glasses are prescribed, they must be brought to and worn during the assessment where required.

During the Assessment

Assessments are conducted professionally and sensitively, with regular breaks as appropriate to the age and needs of the individual being assessed. Assessments may take place over one or more sessions.

Where co‑occurring needs mean that additional accommodations may be required, this should be discussed prior to the assessment. I reserve the right to pause or stop an assessment if the process becomes unduly distressing or if engagement prevents reliable assessment outcomes. If this occurs, next steps will be discussed.

Assessment Location

Available upon request, I can also travel to your home or workplace, subject to a travel fee of 55p per mile from Corston.

 

Where an assessment takes place in a home, school or workplace environment, a quiet room with minimal interruptions and a suitable table and chair is required.  

If the assessment takes place at my home, a parent, carer or responsible adult must be available for children and young people. Adults attending independently must provide an emergency contact.

After the Assessment

Following the assessment, information from formal testing, background questionnaires and observations will be used to produce a comprehensive written report. The report will outline learning strengths, areas of difficulty, conclusions and personalised recommendations for support. A diagnosis will only be made where assessment evidence supports this.

Following a Workplace Needs Assessment, a written report will be provided outlining identified needs and recommended workplace adjustments. Implementation of recommendations remains the responsibility of the employer and/or individual. No guarantee is given that recommendations will be funded or implemented by an employer or third party.

Once full payment has been received, the report will normally be issued within 15 working days of the assessment date. In some cases, further information may be required to support conclusions; if this is necessary, it will be discussed with you, and this may delay the release of the report.

A follow‑up discussion to explain the findings and recommendations can be arranged if requested.

Fees and Report Release

Reports will not be released until full payment has been received. The assessment fee covers the assessment process, professional expertise and report writing and is not dependent on whether a diagnosis is identified.

Cancellation Policy

A £100 deposit is required to secure an assessment booking and is non‑refundable.

If an assessment is cancelled, the deposit will be retained. Where at least five working days’ notice is provided and rearrangement is requested, this will be subject to availability. Due to limited assessment slots, there may be a delay before a new appointment date can be offered.

 

If the individual being assessed is unwell, please provide at least 24 hours’ notice where possible. The assessment will be rearranged at the earliest mutually convenient time, subject to availability.

 

Flourishing Minds Limited reserves the right to cancel or rearrange an assessment due to unforeseen circumstances, including illness. Where this occurs, as much notice as possible will be given and an alternative date will be offered.

Privacy and Data Protection

Flourishing Minds is committed to protecting your privacy and managing personal data responsibly. All information is stored securely and in accordance with GDPR requirements.

Assessment reports and background information will be stored securely for six years after the individual reaches the age of 18, or for adults, six years following the date of assessment. After this time, records will be securely destroyed.

You have the right to request access to your personal data, request corrections, or request deletion where appropriate. Requests should be made to: roanna@flourishing-minds.co.uk
